I dont know exactly what this song means, but I do know that the lyrics and the song mean alot to me.
I think this is about a dysfunctional love where both people hurt each other immensely and yet cannot let each other go. The relationship may even be abusive from both sides toward each other either physically or mentally or both and could be dysfunctional because of drug abuse.

See, recently i lived a situation like the one described in this song...
Me and her, we were in love... everytime, at any moment, we were making promises to each other, that our love would last forever... that we would ALWAYS be together...
Years passed by... things happened, fights happened... she decided to end our relationship, but still, she told me that she would ALWAYS love me...
I tried to get back with her, but, everytime i tried to talk to her, i got vulnerable... and she got more cold towards me, everytime...
I started to hate her, to hate me, even if i still loved her (I LOVE YOU, I HATE YOU)... It's basically the same feeling, they both consume you...
She was rejecting me, and was like.. i felt like i was crawling, begging to her, but... she rejected me, every time (I cant live without you... I cant get around you)...
And you know... every time, i heard her voice, saying that.. "I will ALWAYS love you" (actually, that was the only thing that kept me sane)...
And then...i tried to kill myself. I cut my own wrists. I felt like i was gaining back my dignity, (Does it make you feel more like a man?)...
I survived. And even if still love her (and hate her)... I understood something, and it relates to this song.
This song (and the end of my relation) was all about abuse. Abuse to myself, and abuse to her. Not physically, but emotionally.
About the line, "Does it make you feel more like a man?" Thats the guy telling that to himself. Speaking to his inner self. in a state of extreme loneliness... Then he "wakes up" to reality, and says "the pistol is shaking in my hand"...

I think there are multiple reading sof this song - I've always struggled to pin this one down.
Interpretation #1:
In a similar vein to what Kalyandra said, perhaps the voice is actually a girl's and the song covers the day she decided to change and get herself out of this abusive situation, i.e:

Also, there are refers to running away "I guess I'll pack all my things", "I pick myself off the floor and now I'm done with you"
However, seeing as we know Josey Scott is a GUY, I figured it could be about something else...
Interpretation #2: I figured it could also be a about child abuse, too but this one’s a bit more tenuous, seeing as it refers to "desire" and I'm not even getting into how gross that is >.< However, it could be about a kid watching his dad/mum fight and decides to sort it, as in "why would you tear my world apart"
But I guess you can get anything out of song like this.

he doesnt kill her, he kills himself. he loves her so much but she doesnt like him so he hates her. the blood is on her hands b/c she is responsible for his suicide. i think its pretty obvious

All these meanings posted on this page make a lot of sense, and I agree that battered girlfriend prespective is probably it, but does it confuse anybody else that we left out the most obvious explanation? First time I heard this, I thought maybe he was singing as a guy about a guy. If it weren't for the line "does it make you feel more like a man" would we just have assumed that he was writing, as himself, about a girl. As soon as another man is mentoned, we bend over backwards to find explanations that keep the song straight. Not that I'm calling all of you bigots or anything - you're probably not - it's just a thought.
