This song dylan recorded on live at the gaslight and maybe another album im not aware of. It is originally a dave van ronk who played at the gaslight as well. Dylan really admired van ronk and often played his songs when he was just getting started.

"Cocaine's for horses and it's not for men" is a reference to fixing horse races by giving a horse cocaine to speed it up.

To me, this song is about how, when you find yourself addicted to cocaine, whether you smoke it, inject it, or snort it, it becomes your whole life. Everything you do revolves around cocaine. Its "all around my brain". When he says "hey baby you better come here quick, this old cocaine gonna make me sick" he's talking about craving that high that comes with the drug, to the exclusion of everything else. Near the end he talks about how the doctor tells you that its gonna kill you but he doesnt tell you when, I believe he's referencing the fact that when you're in that vicious cocaine fueled lifestyle, you are very aware of the fact that its going to kill you, but you dont know when and it could happen each and every time you choose to indulge, whether it happens in the form of the police taking you to jail, another crazed addict taking your life, or the meaningless repetitive cycle in itself of living to get high and needing to get high to live.
