Very deep song. Incredible, as we can all agree. This song encompasses true drug addiction, and my interpretation is the song is speaking as the drug.

It's probably saying that things change but stay the same. We don't know who Samantha is except that she's probably not anyone else mentioned in the song. Maybe she's the family's little girl in 1921, baby's older sister. In 1981 she is in her 60s or 70s and observing the same scene.
